Sign Up Forms

How to sign up for Monticello Pro Net. :

  • Call us at 762-3232 and we'll sign you up over the phone with Visa or Master Card.
  • We'll mail you the forms and you can mail them or fax them back with a check or credit card number.
  • Fax the forms to 762-3234
  • E-mail us and we'll mail the forms to you - info@monticellopro.net
  • Print the forms out now in PDF format by clicking here
  • Come into our shop on the Court House Square in Monticello, Illinois between 8:30 am and 5:30 pm to sign up. It usually takes 10-15 minutes.
Get Sign On Forms online!
They're in PDF format so you'll need Adobe Acrobat Reader™ which you may already have on your computer. You can check this on Windows™ machines by going to the Start button, then Programs, then see if Adobe Acrobat Reader™ is listed.
